GeriMedRisk is an interdisciplinary telemedicine consultation and education service for doctors, nurse practitioners and pharmacists in Ontario. Using telephone and eConsult, clinicians receive a coordinated response to questions regarding optimizing medications, mental health, and comorbidities in older adult patients from a team of geriatric specialists and pharmacists. GeriMedRisk is a not-for-profit service for clinicians.

RAAM clinics are accessible to patients without appointments or medical referrals. They offer both medication and psychosocial interventions from the first visit, ensuring a holistic approach to addressing the patient’s substance use. The goal of the RAAM clinic is to meet patients where they are at and work with them to create a care plan that fits with their goals.

TIP (Telemedicine IMPACT Plus) provides rapid access to a virtual team of professionals to enable proactive health and social care for patients with complex conditions, and their family caregivers.

Telemedicine IMPACT Plus is an OHIP-Billable Service that supports coordinated care planning and derives new solutions for addressing the patient’s chronic conditions.  The Primary Care Provider, patient, and their family caregivers benefit from the support of a dedicated nurse who coordinates the patient’s circle of care.
