The Alliance for Healthier Communities membership includes Ontario’s 72 Community Health Centres (CHCs), 11 Aboriginal Health Access Centres (AHACs), 7 Community Family Health Teams (CFHTs) and 15 Nurse Practitioner-Led Clinics (NPLCs). Every one of the Alliance’s members is a not-for-profit, interprofessional, community-governed primary health care organization, committed to the Alliance’s Vision and Mission, including the commitment to Anti-Oppression and the Health Equity Charter.
